① Customers who spend 10,000 yen or more (tax included) in a single transaction at participating stores will receive one application form.

*Applications from multiple stores cannot be combined. *Distribution will end as soon as application forms run out.

②Tear off the application form and drop it in the designated box for the prize of your choice.

*Please be careful not to drop your receipt in the wrong mailbox and not to lose it until the winners are announced.
